If you are Licensed to do business in Mississippi and wish to write business through MRPIUA please complete this form (W-9) submit to [email protected] . This submission should also include the Authorized User Form and a copy of your Mississippi Department of Insurance agency, or producer level active license. The code can be established using an individual or entity level MS Insurance Dept. license.
Please include all of your agency location contact information - primary phone contact #, primary office email contact #, fax #, mailing address for policy output/general correspondence, and mailing address for commission checks (if different).
Note: The W-9 information is used for processing of 1099 tax reporting. If the intent is to set up the producer code at individual license level, the W-9 should provide the individual's SSN. If at entity level, the W-9 should provide the FEIN #.
Once approved, a producer code will be assigned and you will receive a user name and password via email.
